چکیده

INTRODUCTION A Phosphate Centric Forum, supported by Genzyme Corporation, was held on 24–25 June, 2010 at the Sheraton West Park Hotel, Munich, Germany, in which 25 medical and scientifi c experts in the fi eld of chronic kidney disease-related mineral and bone disorder (CKD-MBD) met to present and discuss the central role of phosphate in the development of CKD-MBD, on which research has now celebrated its 50th anniversary. Phosphate plays a central role in the pathophysiology of CKD-MBD and the progression of CKD and contributes to the disproportionate cardiovascular risk faced by patients with CKD. Adaptation of nephrons attempting to preserve phosphate homeostasis requires endocrine tradeoff s that fuel adverse events of hyperphosphatemia in CKD—secondary hyperparathyroidism, calcium and vitamin 50 Years of Research and Discovery in Chronic Kidney Disease and Mineral & Bone Disorder: The Central Role of Phosphate

Vitamin D deficiency in children with Chronic Kidney Disease-Mineral Bone Disorder (CKD-MBD) and factors affecting response to cholecalciferol therapy: A quasi-experimental study from low-middle income setting

Background and Objective: Chronic Kidney Disease-Mineral Bone Disorder (CKD-MBD) is characterized by hypocalcemia, hyperphosphatemia and abnormal vitamin D (VD) metabolism resulting in high parathyroid hormone secretion. The objective of the study was to determine VD status in children with CKD-MBD and the effect of cholecalciferol therapy in these children. Methods: This quasi-experimental st...

Low Bone Mineral Density and Associated Factors in Patients with Cystic Fibrosis: A Cross-Sectional Study

Background: Failure to maintain bone mass density is a major complication in patients with cystic fibrosis (CF). This study was conducted to evaluate the prevalence of low bone mineral density (BMD) and also identifying associated risk factors in CF patients. Materials and Methods: Present study conducted on 59 CF patients aged 5-35 years referred to respiratory clinic of Masih Daneshvari Hospi...

Cardiac valve calcification: an immutable pathologic finding in chronic kidney disease?

Although less frequent than vascular calcification, cardiac valve calcification (CVC) is a relevant clinical problem affecting about 2%-10% of adults from the general population aged 75 years and older, and is 5- to 10-fold more prevalent in individuals with impaired kidney function. An expanding body of evidence suggests that mineral metabolism abnormalities aside from traditional cardiovascul...

The role of fibroblast growth factor 23 in chronic kidney disease-mineral and bone disorder.

Fibroblast Growth Factor 23 (FGF-23) is a bone-derived hormone involved in the regulation of phosphate homeostasis. FGF-23 levels are extremely elevated in Chronic Kidney Disease (CKD) and there is evidence supporting the role of this hormone in the pathogenesis of Chronic Kidney Disease-Mineral and Bone Disorder (CKD-MBD). Furthermore, recent data associates FGF-23 with the pathogenesis of sys...
